TRANSITIONAL HEALTH BENEFIT

If:

  • you are leaving the Ontario Disability Support Program for paid work and
  • you do not have comparable health coverage from your employer,
you and your family may be eligible for the Transitional Health Benefit.

Paid work can be a full-time or part-time job, a training program or running your own business.

Eligible family members include:

  • your spouse, and
  • your children who are under 18 years of age.

Children who are 18 years of age or older are not eligible.

The Transitional Health Benefit covers:

  • prescription drugs
  • basic dental care
  • vision care.

You will continue to receive the Transitional Health Benefit until your employer provides similar health benefits. You must notify your local Ontario Disability Support Program office if your circumstances change. We will also review your eligibility for the Transitional Health Benefit every year.

How do I get this benefit?

Contact your local Ontario Disability Support Program office. Your worker will first determine whether you are eligible for the Extended Health Benefit.

If you are not eligible for the Extended Health Benefit, your worker will determine whether you are eligible for the Transitional Health Benefit. We will send you a letter to tell you if you have been approved.

If you do not qualify for the Transitional Health Benefit

You can apply to the Ministry of Health and Long-Term Care’s Trillium Drug Program for help with your prescription drug costs.

You can get an application for the Trillium Drug Program:

  • at your local pharmacy, or
  • by calling the Trillium Drug Program at 1-800-575-5386.
